The Upper Mustang Trek is a recently opened trekking destination in Himalayan region of Nepal. This trek has been open to foreign trekkers since 1992. Yes, you need the special Upper Mustang permits to trek in the Upper Mustang region. The Upper Mustang is a restricted area due to its proximity to the Tibet Border and to protect the local culture as well as control of trekkers.
When you reach Kagbeni Village, you enter a restricted area, and you should have a Restricted area permit. The Upper Mustang Restricted Area Permit costs USD 500 per person for the first 10 days. It is not possible to trek independently in Upper Mustang because, according to the rules and regulations of the Nepal Government, you now need a guide.
